Evaluation Cooperative Education

The evaluation of the student’s performance on the out-of-school components will be shared between the employer and the co-op teacher(s).

Employer Evaluation of Student -- 40%
Mid-term Evaluation (Yellow Form)
Final Evaluation (Pink Form)

Written Assignments and Projects -- 30%
Employment Preparation—resume, cover letter,
application form, interview questions, safety assignment.
Personalized Placement Learning Plan
Culminating Activities

Activity Sheet and Weekly Journals -- 30%
Accurate account of activities and hours
Completed journals on topics assigned during in-school sessions

 

EMPLOYER’S EVALUATION AT WORKSITE:

The employer will formalize his/her evaluation of the student through:
a. A mid-term evaluation given to the place of work when the student has completed approximately 110 hours of work.
b. A final evaluation form will be completed by the employer when the student has completed 220 hours of work.

COOPERATIVE EDUCATION TEACHER’S EVALUATION:

The co-op teacher will evaluate the student’s performance through:
a. Monitoring visits to the job placement to interview the employer and the student.
b. The weekly activity reports and weekly journal entries submitted by the student.
c. Culminating Assignments

Policy for Submission of Weekly Activity Reports and Journals

Students will submit a weekly activity sheet and journal, signed by their employer, EVERY MONDAY worth 20 marks each.

NOTE: In addition to the above marking scheme, the student must complete 220 hours of on-the-job-training in order to qualify for two co-op education credits. Failure to complete the hours will result in credit loss.
